Can the person booking a court sign for the whole group?

Use individual participant records under your signing policy. The person paying for a court is not automatically authorized to accept participation terms for the other adult players.

Do regular members need to sign at every visit?

Set a renewal policy with your reviewers and apply it consistently. Staff should check that the record covers the relevant program and period rather than asking regulars to repeat forms without a reason.

Does the waiver assign a player's skill rating?

No. You may collect experience for clinic planning, but ratings and court placement belong to your club's separate play-management process. Signing confirms the participation agreement.
