Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Member or guest identity

Collect the person training, their contact details, and whether they hold a membership, trial, or day pass. A purchaser's name alone is insufficient.

  2. 02

    Training areas and access

Name the weight room, cardio area, and other included spaces. Explain staffed and unstaffed access only where those arrangements actually exist.

  3. 03

    Exercise and equipment risks

Describe lifting, moving weights, machine adjustments, and exercise-related exertion. Leave space for your reviewed assumption-of-risk and release language.

  4. 04

    Floor rules and reporting

Set expectations for returning weights, asking for equipment help, keeping walkways clear, and reporting damaged equipment to the designated staff member.

  5. 05

    Emergency contact and assistance

Record a reachable emergency contact and explain how members request assistance, including the procedure during any unstaffed hours.

  6. 06

    Participant agreement

Capture the appropriate participant or guardian signature and date. Keep payment authorization and recurring membership charges clearly identified in their own agreement.

Make arrival easier for your team.

  1. 01

Place a QR code at day-pass check-in so walk-ins can sign on their own phones

  2. 02

Check each guest separately when a member brings a training partner

  3. 03

Record completion in your existing membership workflow before issuing access credentials

Make it specific to your operation.

  1. 01

List the training areas and access hours covered

  2. 02

Define the guest and minor-member signing process

  3. 03

Match equipment and assistance instructions to actual staffing

  4. 04

Specify when members need an updated waiver

  5. 05

Have counsel and your insurer review the release and access terms

Review before you publish.

Can members and day-pass guests use the same gym waiver?

Yes, if the document accurately describes both arrangements. Include a visit-type field and explain any different access rules so staff can distinguish a recurring member from a one-day guest.

Does signing the waiver activate a key fob?

No. Staff should verify the submission and complete the gym's separate access process. FlexWaiver collects signed records; access-control behavior depends on your own systems and procedures.

Should a gym waiver include personal training?

Include it only when the language covers that service and the responsible trainer. One-to-one coaching may need additional session details, especially when an independent trainer works inside your facility.
