Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Swimmer and lesson booking

Record the swimmer's name, age, lesson dates, and group or private format. Distinguish the child participating from the adult making the booking.

  2. 02

    Pool location and activities

Name the pool and skills taught, such as floating, water entry, and stroke practice. State whether an accompanying adult participates in the water.

  3. 03

    Water-related risks

Explain submersion, drowning, slips, and pool contact in reviewed risk language. Avoid describing lessons or a signed form as proof of water safety.

  4. 04

    Experience and instructor conversation

Ask about previous lessons and comfort in the water to inform an initial conversation. Keep skill placement subject to the instructor's actual observation.

  5. 05

    Supervision and poolside handoff

Explain when the lesson starts and ends, where guardians wait, and who supervises outside instruction. Describe how swimmers are returned to their responsible adult.

  6. 06

    Emergency details and signature

Collect the appropriate contact and participant or guardian signature. If a host facility requires its own form, identify that separate signing step.

Make arrival easier for your team.

Have counsel and your insurer review aquatic and guardian terms

Review before you publish.

Can parents report their child's swimming level on the waiver?

Yes, as background information. Describe it as parent-reported experience; the instructor still observes skills and determines appropriate lesson activities through the school's usual process.

What if lessons take place at a hotel or community pool?

Name the location and confirm the host's requirements. Families may need both your instruction agreement and a facility form, so explain the two steps before the first visit.

Should the form cover parent-and-child lessons differently?

Yes. Describe the adult's in-water role and collect the appropriate participation information. A guardian signing for a child and an adult entering the lesson are different circumstances.
