Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Camper and session identity

Connect each child to the camp location, dates, age group, and enrolling guardian. Keep siblings individually identifiable on the daily roster.

  2. 02

    Activities and separate permissions

List the actual camp program. Make swimming, off-site trips, transport, or overnight stays explicit when offered rather than implying blanket permission.

  3. 03

    Camp activity risks

Describe outdoor play, group games, terrain, tools, and exposure relevant to your schedule, with language families can understand before enrollment.

  4. 04

    Care and emergency contacts

Identify reachable adults and the route for sharing allergies or support needs. Keep medication instructions in your approved camp care process.

  5. 05

    Attendance and handoff expectations

Explain check-in, authorized pickup, behavior expectations, and schedule changes. Reference the current parent handbook so staff and families use matching procedures.

  6. 06

    Guardian consent and signature

Include your reviewed participation and release language, identify the signing adult's relationship, and keep optional photography permission clearly distinguishable.

Make arrival easier for your team.

  1. 01

Compare signed records with the camper roster before each new session begins

  2. 02

Resolve missing guardian consent before the child joins the activity group

  3. 03

Brief counselors on actionable permissions through your established roster process

Make it specific to your operation.

  1. 01

Specify day camp or overnight scope and the exact session dates

  2. 02

Identify activities requiring their own permission or provider documentation

  3. 03

Align pickup wording with the handbook and staff handoff process

  4. 04

Give guardians a clear way to communicate changed care needs

  5. 05

Ask counsel and your insurer to review camp and guardian language

Review before you publish.

Can one summer camp waiver cover every week?

Define the period and activities in your reviewed wording. Recheck enrollment when campers add weeks or choose a different program; an earlier signature does not explain newly introduced activities.

Can the adult dropping off a child sign?

A carpool driver or relative may be authorized for pickup without authority to provide guardian consent. Establish your signing requirements and collect the appropriate guardian's signature before arrival.

Should medication instructions go in the waiver?

Use the camp's approved health and medication documentation. The waiver can point families to that process, but a general participation acknowledgement should not stand in for specific care instructions or authorization.
