Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Athlete and visit details

Identify the athlete, class date, and foundations, member, or drop-in status. Collect experience context without treating a stated skill level as coach approval.

  2. 02

    Workouts and training spaces

Describe barbell work, rig movements, conditioning, and any outdoor running. Specify whether supervised classes and open gym are both included.

  3. 03

    High-effort exercise risks

Explain fatigue under load, missed lifts, falls, and contact with equipment. Add reviewed assumption-of-risk and release language for your operation.

  4. 04

    Scaling and coaching directions

Set expectations for asking about unfamiliar skills, using agreed modifications, and stopping when needed rather than prioritizing a score or prescribed load.

  5. 05

    Shared floor responsibilities

Explain lifting lanes, rig spacing, equipment return, and the process for reporting a damaged bar, box, ring, or other training item.

  6. 06

    Emergency details and signature

Collect a reachable emergency contact and the athlete or guardian signature. Keep optional workout photographs separate from participation consent.

Make arrival easier for your team.

  1. 01

Check drop-in submissions before the class briefing starts

  2. 02

Give visiting athletes a clear place to ask about local equipment rules

  3. 03

Review your waiver when adding a new training area or materially different program

Make it specific to your operation.

  1. 01

List the movements and spaces your gym uses

  2. 02

Explain foundations requirements and drop-in expectations

  3. 03

Address open gym separately if coaching availability differs

  4. 04

Name your own business in the reviewed release terms

  5. 05

Confirm youth-program wording with counsel and your insurer

Review before you publish.

Should experienced drop-ins still complete our gym's waiver?

Follow your own signing policy for visitors. Experience at another gym does not establish agreement to your facility's terms, and your coach still needs a brief introduction to the athlete.

Can the waiver approve an athlete for advanced movements?

No. A form can record previous experience, but coaches make participation and progression decisions through the gym's usual instruction and assessment process.

Is this an official CrossFit waiver?

No. It is an independent starting structure for operators offering CrossFit training. It does not imply affiliation or approval; adapt it to your business and obtain your own legal review.
