Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Boxer and class details

Identify the participant, age, class date, and experience. Record the booked format so a bag-class reservation is not mistaken for a sparring session.

  2. 02

    Training format and contact

Describe shadowboxing, bags, mitts, partner drills, and any sparring separately. State clearly when a program has no intentional participant-to-participant striking.

  3. 03

    Boxing-specific risk terms

Explain hand and wrist loading, moving equipment, and fatigue. Where contact is offered, include its distinct head and body impact risks in reviewed language.

  4. 04

    Equipment and floor conduct

Set expectations for wraps, gloves, protective equipment, bag spacing, and ring access. Explain how participants request a fit or equipment check.

  5. 05

    Stopping and reporting concerns

Explain the coach's stop signal and how participants report discomfort, an impact, or damaged gear. Connect these acknowledgements to your actual incident procedures.

  6. 06

    Emergency information and consent

Collect emergency contacts and participant or guardian signatures. Identify any separate documentation your contact or competition program requires.

Make arrival easier for your team.

  1. 01

Place QR signing near reception rather than beside active bags

  2. 02

Verify the youth participant's name as well as the guardian's signature

  3. 03

Review consent requirements when a fitness client asks to move into contact training

Make it specific to your operation.

  1. 01

State whether each program is fitness-only or includes contact

  2. 02

Specify required personal and loaned protective equipment

  3. 03

Describe rules for entering the ring and joining partner drills

  4. 04

Align injury-reporting language with your established procedures

  5. 05

Review contact-program terms with counsel and your insurer

Review before you publish.

Does a fitness boxing waiver cover sparring?

Do not assume it does. Sparring changes the activity and contact exposure. Review the language and the participant's eligibility before adding it to a bag-work or conditioning program.

Should loaned gloves be mentioned?

Yes. Describe how participants collect, check, and return loaned equipment. Give them a clear way to report fit or condition concerns before class begins.

Can this form replace competition paperwork?

No. This is a gym-intake starting point. A sanctioned bout or event may have its own organizer, eligibility, and documentation requirements that must be handled separately.
