Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Rider and session identity

Collect the rider's name, age, visit date, and lesson or event group. Identify the responsible guardian separately from a coach, friend, or team organizer.

  2. 02

    Terrain and permitted disciplines

Describe the bowls, ramps, rails, stairs, or street features included. List the equipment permitted in this session and any restricted areas or discipline-specific times.

  3. 03

    Riding and terrain risks

Explain falls from features, failed landings, moving equipment, and collisions where riders' lines intersect. Describe surface changes relevant to indoor or outdoor use.

  4. 04

    Turn-taking and park conduct

Set expectations for checking a line before dropping in, waiting clear of landings, and obeying staff closures. State which tricks or equipment are restricted during lessons.

  5. 05

    Protective equipment and instruction

Insert the park's helmet, pad, and equipment-condition requirements. Explain where beginners can ask for instruction and how riders report damaged features or equipment.

  6. 06

    Guardian consent and signed agreement

Add reviewed participation terms and the appropriate signature. State youth supervision and pickup arrangements for coached programs without implying open sessions include personal coaching.

Can one template include boards, scooters, and BMX?

It can if your park permits them and the activities and rules are described accurately. Separate versions may be clearer when equipment types use different sessions, features, or protective requirements.

Should a contest use the everyday open-session form?

Review the event format first. Timed runs, designated practice, spectator areas, and restricted course access may require event-specific instructions and an updated agreement.

Does signing establish a rider's skill level?

No. Collect experience information only when staff use it for lesson placement or access decisions. Instructors and park staff should still explain the appropriate features and assess readiness through their normal process.
