  1. 01

    Client and session details

Identify the client, first session, and private or group format. Ask about prior apparatus experience to support the instructor's opening conversation.

  2. 02

    Mat and apparatus activities

Name the equipment used, such as reformers, towers, chairs, and small props. Include only apparatus offered in the covered program.

  3. 03

    Movement and equipment risks

Explain balance changes, spring tension, moving surfaces, and exertion. Place your reviewed risk and release terms alongside these concrete descriptions.

  4. 04

    Setup and instructor cues

Explain who changes springs and footbars, how clients mount equipment, and when to wait for assistance rather than copying another participant.

  5. 05

    Participation preferences and concerns

Provide a route to discuss movement adjustments and hands-on instruction preferences. Make clear that clients can ask to pause or decline a cue.

  6. 06

    Emergency contact and agreement

Collect the emergency contact and participant signature. Label studio booking policies and optional photographs independently from the activity agreement.

Review before you publish.

Can mat and reformer Pilates share a waiver?

They can if the document clearly describes both. A mat-only form may omit moving carriages and spring equipment, so review the language before extending it to apparatus classes.

Should hands-on adjustments be included in the signature?

Explain your teaching approach and give clients a clear way to state their preference. A signed form should support an ongoing conversation; preferences can change during class.

Does the waiver replace a reformer introduction?

No. Staff still need to show new clients how your specific apparatus works. Use the signed record to finish intake, then leave time for equipment setup and questions.
