List the equipment and activities included, such as bars, beam, vault, floor, tumble track, or foam pits. Describe open gym separately from lessons.

Instruction and station rules
Explain waiting for a coach, following progressions, keeping landing zones clear, and using only assigned stations rather than independently trying a new skill.

Questions to settle
Review before you publish.
Can a class waiver also cover open gym?
Review the actual activities and supervision arrangements first. Open gym may allow different station use or age groups, so its terms should be explicit rather than assumed from a weekly lesson.
How should siblings appear on the records?
Ensure each gymnast can be identified by name, age, and class. Staff should be able to verify the child arriving without relying only on a shared guardian name.
Does signing establish a gymnast's skill level?
No. The form can collect experience notes, but coaches assess placement and skill progression. Keep waiver verification and instructional decisions as distinct steps in first-class intake.
