Complete waiver
Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Participant and event identity

Name the child or adult participant, guardian where needed, event date, and location. Keep the party host's rental details separate from individual participation records.

  2. 02

    Inflatable activities included

List the bounce unit, slide, obstacle course, or other equipment actually provided. State whether water use is included and which attractions require different rules.

  3. 03

    Bouncing and shared-play risks

Describe unstable footing, falls, collisions, and entry or exit hazards. Address mixing users of different sizes and movement near slide landing zones.

  4. 04

    Attendant and participant rules

Insert capacity, size-group, footwear, prohibited-item, and climbing rules from your operating procedures. Explain who controls entry and when participants must leave the unit.

  5. 05

    Supervision and stop-use instructions

Clarify guardian and attendant responsibilities. Tell guests how staff respond to weather changes, loss of inflation, or other stop-use conditions without assigning equipment inspection to children.

  6. 06

    Guardian consent and signature

Collect the appropriate consent and reviewed release wording for the named participant. Provide an emergency contact reachable during the event and record the signature date.

Make arrival easier for your team.

  1. 01

Give hosts a participant link they can include with invitations

  2. 02

Place the QR code before the attraction entrance rather than on the unit

  3. 03

Pair signature verification with the attendant's size-group and capacity checks

Make it specific to your operation.

  1. 01

Use the manufacturer's limits and your equipment-specific operating rules

  2. 02

Distinguish participant consent from the separate rental and setup agreement

  3. 03

Identify who supplies attendants and supervises children at this event

  4. 04

Describe wet attractions separately from dry bouncing activities

  5. 05

Have counsel and your insurer review the completed wording

Review before you publish.

Does the rental customer's signature cover every child?

Do not treat a rental contract as individual guardian consent. Arrange a participant-signing process for invited families and decide who checks completion before children enter the inflatable.

Should a water slide use the same form as a bounce house?

Only when its activities and rules are clearly covered. Water, slide entry, landing areas, and clothing requirements call for different details from a dry bounce-only unit.

Where do anchoring and weather inspections belong?

Keep them in the operator's setup and monitoring procedures. The participant form can explain stop-use instructions, while trained staff remain responsible for inspecting the installation and applying equipment limits.
