Illustrative signing preview. Your published form reflects the fields and wording you choose.
  1. 01

    Volunteer and project identity

Record the volunteer's name, organization, project location, date, and role. Keep the assigned task visible so consent refers to the actual work planned for that shift.

  2. 02

    Activities and role boundaries

Describe cleanup, sorting, lifting, visitor assistance, or other duties. State which tasks require training or separate authorization, such as driving, power tools, or work at height.

  3. 03

    Project-specific risk acknowledgement

Explain the conditions volunteers may encounter, including uneven ground, sharp waste, repetitive movement, or outdoor weather. Remove hazards unrelated to the assigned project.

  4. 04

    Supervisor instructions and equipment

Name the reporting contact and describe required clothing, protective equipment, and stop-work procedures. Explain how to report an unsafe condition or request another task.

  5. 05

    Emergency contacts and youth participation

Collect an emergency contact and guardian consent for approved youth roles. State minimum ages, adult supervision, and check-out arrangements for the particular project.

  6. 06

    Consent and signature

Add reviewed participation and release wording with the signature date. Keep optional photo permission and volunteer mailing-list choices distinct from agreement to the assigned work.

Is this the same as a volunteer application?

No. Applications gather interests, availability, or qualifications. This blueprint focuses on consent for a defined project after volunteers know the work and conditions they are being asked to accept.

Can one form cover every role at a nonprofit?

Use a common structure where appropriate, but describe the actual duties. Donation sorting, neighborhood cleanup, driving, and construction can require different instructions, training, and reviewed terms.

Should photo permission be required to volunteer?

Keep promotional-image permission a clear, separate choice and establish how staff honor it. Participation consent should remain understandable without bundling unrelated communications or publicity preferences.
